Commissioner of Oath Services are available by appointment only at the Municipal Offices (395 Mulock Drive, Newmarket)

The Commissioner of Oaths will sign affidavits or statutory declaration documents including the following:
- Sworn statement for family gift of a used motor vehicle
- Domestic and Foreign pension document (Proof of Life)
- Application to change a name
- Declaration of Common Law or Single Status
- Statutory Declaration for Ontario Student Assistance Program (OSAP) – Statement of Common Law Status/Supporting Children
- Statement of Conscience or Religious Belief (Immunization Exemption Document)
- Declaration of Unregistered Vehicle
- Statutory Declaration of Construction Completion Form (CCDC)
- Statutory Declaration in Lieu of a Guarantor (Canadian Passport, specialized document)
- Affidavit of Proof of Loss of Motor Vehicle/Boat (from an insurance company)
Town of Newmarket Commissioners of Oath do not have authority to sign any of the following documents:
- Notary Public Documents
- Estate, Will or Power of Attorney documents
- Documents related to finance (banking information, RRSP, etc.)
- Real Estate documents
- Documents that are leaving Ontario
- Divorce, separation, marriage or cohabitation agreements
- Consent letters for children travelling abroad
- Letter of Invitation for visitors to Canada
- Certified True Copies
- Any court related documents
At your appointment, you must present one original, valid piece of government issued photo identification (no copies permitted). Examples include: Driver's License, Passport, or Photo Identification Card
The Town does not provide Notary Public services. A list of Notary Publics and lawyers who notarize documents can be found in your local phone directory.
Fees:
Residents of Newmarket: $26.32
Non-Residents: $28.95
Fees are charged per document. There is no fee for Domestic and Foreign pension documents (Proof of Life).
